I understand, accept and agree that if an amount is uncollectible as a result of the indicated credit card being canceled, the charge being placed in dispute, or otherwise, then any payments applied by the City of Chicago will be reversed and the debt will remain outstanding, and I will pay to the City of Chicago a credit card nonpayment fee of $34.

I also understand, accept and agree to pay any and all fees and/or penalties associated with the submission of an Online Check payment transaction that is returned by the bank or other depository financial institution for insufficient funds or any other error on my part, including an insufficient funds check processing fee of $34 payable to the City of Chicago.

I also agree and stipulate that I am the account owner or authorized user of the bank or other depository financial institution account information I have provided for the purposes of making a payment to the City of Chicago.
