You will keep your username, password and answer to the secret question secure. These three elements together comprise your unique log-in data and your unique electronic signature, referred to on the Home Page of this ELF site. Your electronic signature is a necessary and legally required part of your LOBBYIST FILINGS and of the procedure for successfully filing electronically your LOBBYIST FILINGS with the Board. By electronically submitting - or re-submitting a deficient or amended LOBBYIST FILING, you will be verifying your electronic signature, the truth and completeness of your LOBBYIST FILING, and your continuing agreement to these Terms and Conditions, as instructed on the "Ready to Submit" page in this web application. When you so submit, your verification as to your LOBBYIST FILING content and filing is as follows: "I declare, affirm and represent that I have examined the information in this STATEMENT OF REGISTRATION, AMENDMENT THERETO, ACTIVITY REPORT OR NOTICE OF TERMINATION, including accompanying or supplemental documents or statements, and certify that to the best of my knowledge, information and belief it is true, accurate, correct and complete. I understand that knowingly filing a LOBBYIST FILING containing false or misleading information can result in sanctions under the City of Chicago's Governmental Ethics Ordinance, including fines or suspension of registration, in accordance with applicable rules, regulations and ordinances or the City of Chicago." Your use of this electronic filing application in no way changes the applicability of the Governmental Ethics Ordinance to you as a registered lobbyist under Chapter 2-156 of the Municipal Code of Chicago or the City of Chicago Board of Ethics Amended Rules and Regulations.
