To request a video message from the City of Chicago Mayor please complete all of the information below. If the Mayor was previously scheduled to participate in the event please make note.

Applications will be reviewed and approved based on availability of the Mayor. If approved, a draft script or talking points will be requested to create a script. Completed videos will be e-mailed or submitted as noted in application. All requests are subject to change.
