CN011012 Failed to provide self-closing, framed, 1-1/2 hour Class B fire door or other approved opening protective assembly for heating plant and/or boiler room. (15-8-230) The basement boiler enclosure has a plywood door. Install a class B fire door. Section 14B-7-716.
CN039013 Failed to post in a conspicuous location, the approved occupancy card indicating the number of people who may legally occupy such rooms and space in every theater, public assembly unit or open air assembly unit and in every room or portion of such units. (13-84-410) The bar and the patio have no occupancy placards. If the 2cd floor event space becomes licensed and approved for use, a placard is required in that space as well.
CN061064 Make repairs to building rapidly becoming dangerous, and to bring and maintain in good and safe condition, or wreck part that endangers life and property. (13-12-130, 13-8-100) The bar and the 2cd floor event space have been issued DOB / BACP closures. There are Dangerous and Hazardous egress items as well as extremely Dangerous and hazardous electrical violations; So much so that a ComEd disconnect order has been issued. There are also numerous plumbing violations pending that are not in this report. Section 14XA-3-315.
CN069014 Failed to maintain foundation in sound condition and repair and so it adequately supports the building at all points. (13-196-530, 13-196-530(a), 13-196-641) The masonary basement foundation is degraded with much moisture penetration causing severe effloressence. section 14X-3-303.
CN101015 Failed to maintain interior walls and ceilings free from holes or cracks. (13-19-540(c)) There is a large hole in the 2cd floor ceiling.
CN106015 Failed to maintain interior stairway system in safe condition and sound repair. (13-196-570) Behind the bar is a set of stairs to the basement. Theree is no bannister at the top and no hand rails on the stairs. There is a hazard of the bar tenders stepping back and falling into the basement. Dangerous and Hazardous. Section 14X-3-304.
EL0019 Replace defective light fixture. (18-27-410.22, 18-27-410.23, 18-27-410.24, 18-27-240.27, 18-27-410.36, 18-27-410.37, 18-27-410.38, 18-27-410.39) The basement and 1st floor bar have numerous defective light fixtures. Dangerous and Hazardous. section 14X-7-701.
EL0029 Remove exposed wiring. (18-27-300.4) From the basement to the 1st floor there exists numerous locations of exposed, live, electrical wiring. Extremely Dangerous and Hazardous. Section 14X-7-701.
NC2011 Performed or allowed work to be performed without submitting plans prepared, signed and sealed by a licensed architect or registered structural engineer for approval and without obtaining a permit to perform the work. (13-32-010, 13-32-040, 13-40-020, 13-12-050) The 2cd floor above the licensed bar has an event space that is not licensed. Plans and permits are required to obtain a PPA license for the 2cd floor space. The rear patio was installed without plans or permits. This patio space has an expired license. Section 14A-4-404.
